The size of a pituitary tumor does not predict headache symptoms, according to the results of a prospective study published in the May issue of the Archives of Neurology. Rather, headache was associated with family history.
"Pituitary tumors are commonly associated with disabling headache. The accepted mechanisms for headache are dural stretch and cavernous sinus invasion," write Miles J. Stereotactic Radiosurgery for Pituitary Tumors***
Pituitary adenomas frequently pose challenging clinical problems. Stereotactic radiosurgery (SRS) is one treatment option in selected patients. The purpose of this report is to identify the advantages and disadvantages of radiosurgery in cases of pituitary tumors to assess better its role in relation to other treatment. Methods for optimizing outcome are described. The author reviews several recent series to determine rates of growth control, endocrine response, and complications. In general, growth control is excellent, complications are very low, and reduction of excessive hormone secretion is fair. Depending on the clinical situation, SRS may be the treatment of choice in selected patients....

Treatment of Patients With a Pituitary Adenoma: One Clinician's Experience (By Dr. Mary Lee Vance) ***
The diagnosis and treatment of a patient with a pituitary adenoma or another type of pituitary lesion requires a team approach involving the neurosurgeon, endocrinologist, primary care physician, and, in some patients, an ophthalmologist and radiation therapist. The diagnosis of excessive pituitary hormone secretion and/or loss of pituitary function is based on biochemical findings and can usually be confirmed by performing blood tests, except for patients with suspected Cushing syndrome. The primary treatment of pituitary adenomas is resection, except in patients with a prolactin- producing tumor, who are most successfully treated medically with a dopamine agonist. In this article the author reviews the principles of diagnosis and treatments and offers recommendations for ideal patient care....

Objectives
Pituitary adenomas are the most common type of pituitary tumor. They cause a variety of disorders including acromegaly (excess growth hormone), Cushing's disease (excess ACTH), elevated prolactin (prolactinoma), and rarely elevated thyroid stimulating hormone (TSH). Craniopharyngiomas and Rathke's cleft cysts are other lesions that arise in the pituitary.

Cushing's Syndrome results from chronic exposure to excessive levels of glucocorticoids: its investigation and management remains an on-going challenge in clinical endocrinology. Although the condition is considered rare, the clinical spectrum of the disease is broad, and its investigation is frequently required given the high prevalence of many of its non-specific symptoms such as obesity, muscle weakness and depression. Clinicians are now considering the diagnosis in its earlier manifestations, often before the development of the more classic signs and symptoms as described by Harvey Cushing early in the last century. In its severe form and when untreated, the metabolic upset of Cushing's syndrome is associated with a high mortality, approximately 50% at five years. However, more subtle excesses of cortisol may have significant effects on glycaemic control and blood pressure, and may therefore be an important cause of morbidity....
